Personal information on this form is collected under the authority of the Municipal Freedom of Information and Protection of Privacy Act (MFIPPA), s. 28 (2), and will be used to determine qualification for receiving gas infrastructure mapping information from the City of Kitchener.  Please note that your personal information will be stored in a Third Party server located in Waterloo, Ontario.
