Duct-type smoke detector report

As per the Ontario Building Code, I hereby certify that the duct-type smoke detector has been installed and will turn off the fuel supply (with the absence of power) and electrical power to the heating system upon activation of such detector at the above address as required under Division B, 6.2.4.7.(14) of the OBC.

COMPLETED FORM SHALL BE PROVIDED TO THE INSPECTOR PRIOR TO OCCUPANCY INSPECTION. 

Inspection by a Municipal Building Official does not relieve the owner or contractor of any responsibility to comply with all relevant By-laws, codes, regulations and legislation.
