This form does not include bookings for:

• the Conrad Centre for the Performing Arts
• sportsfields
• ice time at arenas
• Huron Community Centre (please contact staff directly)
• Kitchener Market team building

Your personal information is being collected under the authority of the Municipal Act s.11(1) and in accordance with s.28(2) of the Municipal Freedom of Information and Protection of Privacy Act (MFIPPA). Your information will be used to administer your request for the booking/rental of a City facility. Only City of Kitchener staff who require access to your personal information to facilitate your booking will have access to it.
