Completed forms must be submitted to the office of the mayor and council at least 3 (three) weeks prior to the day requested for raising the flag and/or illumination.
As per the City of Kitchener flag and illumination policy:
Personal information is being collected under the authority of s.11(1) of the Municipal Act and in accordance with s. 28(2) of the Municipal Freedom of Information and Protection of Privacy Act (MFIPPA) and will be used to confirm community flagpole booking and issuing of flags and flagpole crank. This information may be shared with City Staff who require it for their work as part of the Corporation. The personal information collected herein will be used only for the purposes relating to the program and will be protected in accordance with the requirements stipulated in the MFIPPA.
